Support Coordination

Someone in your corner to make sense of the plan, find the right providers and sort things out when they go sideways.

Introduction

What this support actually looks like

A support coordinator helps you turn a plan document into supports that actually run. That means explaining what each budget covers, finding providers with real capacity, setting up service agreements and following up when something is not working.

We work with you, not around you. You choose the providers; we do the ringing around, the comparing and the chasing. If you would rather learn to do it yourself, we will teach you that instead — the goal is for you to need us less.

Who this may suit

  • You have just received your first NDIS plan
  • Your plan has changed and you are not sure what it now covers
  • You are juggling several providers and it has become confusing
  • A provider is not delivering what was agreed
  • You are preparing for a plan review and want it to go well

What is included

What support can include

Support is built from these elements in whatever combination suits you. Nothing here is compulsory.

Understanding your plan

A plain-English walk-through of every budget and what it can pay for.

Finding providers

Options with real capacity, compared honestly — including our competitors.

Service agreements

Reviewing what you are signing before you sign it.

Coordinating the team

Keeping providers, health professionals and family talking to each other.

Sorting out problems

Stepping in when a service is not delivering.

Plan reviews

Evidence, reports and preparation so the review reflects reality.

In practice

Example activities

Real examples of how participants use this support week to week.

  • A two-hour first session mapping your plan against your goals
  • A shortlist of three therapy providers with waitlists and rates compared
  • Chairing a meeting between your OT, your SIL house and your family
  • A written report and evidence pack ahead of your plan review

How it works

From first call to first shift

The same four steps for every service, so you always know where things are up to.

01

Get in touch

Call us or send the request form. We will ask what you need and check we can deliver it.

02

Meet and plan

A home visit to talk through routines, preferences and worker matching.

03

Agree the detail

A written service agreement with days, hours, rates and who your contact is.

04

Start and review

Support begins, with check-ins at two and six weeks to adjust anything.

One business day, and sooner when something is urgent.

Builds your capacity

We show you the how, not just do it for you.

Questions

About support coordination

No. It is funded under Capacity Building and has to be included specifically. If it is not in your plan and you think you need it, we can help you build the case for your next review.

Yes, and most of our coordination participants do. Coordination is deliberately kept separate from service delivery so the advice you get is genuinely independent.

Plan management handles the money — paying invoices and tracking budgets. Support coordination handles the supports — finding them, setting them up and keeping them working. Many participants have both, from different organisations.
